2009 Independent Investigator Registration Form
Registration is MANDATORY for all individuals wishing to apply to the 2009 Independent Investigator Competitions.
Please note that institutional sponsorship remains a requirement for the submission of this form.
New for the September 2009 Independent Investigator Competitions, the Foundation has moved to an online Registration Form through the development of an AHFMR Independent Investigator Registration Portal. Please find more information and a link to access the registration portal below.
General Instructions for Completing and Submitting the 2009 Independent Investigator Registration Form
- Please read through the Independent Investigator Guidelines.
- If you are unsure of the eligibility requirements to apply for an AHFMR Independent Investigator award, an establishment grant or the category of award to apply for, please find more information on our website and/or contact the Director of Grants and Awards, Dr. Pamela Valentine at pamela.valentine@ahfmr.ab.ca or (780) 423-5727.
- Please note that for the September 15, 2009 competitions, AHFMR will only accept applications on behalf of the following Investigators:
- Investigators who are currently supported through an AHFMR Investigator award (Clinical Investigator, Population Health Investigator, Scholar, Senior Scholar, or Scientist) which expires on June 30, 2010.
- Investigators supported at the time of the competition (September 15, 2009), on AHFMR Terminal Awards and who are eligible for a second application.
- Investigators who are not currently supported by AHFMR Investigator Awards (new Investigators) and who are deemed (by AHFMR) eligible to apply in the September 2009 competitions
- To register the Foundation will require:
- A. Online submission of the 2009 Independent Investigator Registration Form through the portal available at 2009 Independent Investigator Registration Portal;
- B. Receipt of the signed original 2009 Independent Investigator Registration Form to the Foundation at the following address:

- DEADLINE for receipt of submission of the Registration Form, including the signed original 2009 Investigator Registration Form is Friday, July 3, 2009 at 4:00 p.m.
- Please note you may be contacted in order to confirm your eligibility to apply to the Independent Investigator Award program. AHFMR is responsible for the final determination of eligibility for this competition.
- Full applications to the Independent Investigator Award program will only be accepted from registered individuals.
