You, and your references may complete the form online. Once it is completed, save it in your computer files, print it, and collect the appropriate signatures. Don't forget to attach your essay. Collect your references in sealed envelopes and mail the whole package to Alberta Innovates - Health Solutions. The application package must be postmarked by March 23, 2012. If you are mailing last minute, make sure the post office clerk stamps your envelope so the postmark is visible. We will accept applications which are completed online AND those completed by hand. If you complete an application by hand, please use ink and write legibly. |
